Together our staff is building a warm and welcoming organizational culture that celebrates our unique perspectives and urges all to participate and provide feedback to ensure continuous improvement.Job Scope: Transition Assessment Specialist provides outreach, engagement, and assessment activities (counseling services) on-site at area nursing facilities to support comprehensive Colbert/Williams transition services. Develop and maintain relationships with area facilities and provide intensive follow-up outreach and engagement efforts. Provide outreach to potential class members through engagement and education. Complete intensive, comprehensive assessments (counseling services) through interviews with the individual, collateral information from the nursing facility, reviewing facility records, requesting documentation from previous providers, and speaking with family members, as appropriate. Assessments are strengths-based, client-centered, and strong engagement and motivational interviewing skills are recommended.Proof of COVID-19 Vaccination RequiredAccommodation for Medical and Religious Exemption will be ConsideredWeekly Testing Required for Approved Vaccination ExemptionsEducation, Experience and Other Requirements

  • Masters Degree from an accredited College or University in Social Work, Psychology, Counseling or closely related discipline
  • Qualified Mental Health Professional (QMHP) under Illinois Rule 132/140 or LPHA/LCSW/LCPC; or RN and OT (Occupational Therapist) Illinois license
  • Preferred 2 years of experience working with adults with serious mental illness and chronic health conditions and 1 year completing comprehensive assessments
  • Preferred 1 year experience working in non-traditional treatment environments, residential, nursing facility, hospital, community settings
  • Applicant must have a valid IL Driver's License, proof of insurance, and daily access to a reliable well maintained vehicle
  • Computer and smartphone proficiency, including working knowledge of MS Word, Excel, Internet skills, text messaging. Knowledge and ability to learn and effectively utilize electronic internal collaboration and communication tools (ex. Microsoft Teams, Microsoft 365)
  • The employee is required to travel by automobile to and from Mt. Prospect, and Elk Grove office locations on a weekly basis, as well as daily client visits in nursing home facilities and SMHRF in the Chicagoland area, including suburban Cook and Lake County, and occasional meetings/trainings in and around the Chicagoland area.
  • While conducting external KYC business or traveling between sites, the employee may be exposed to fumes or airborne particles as well as changing weather and road conditions.
  • Full Time, Exempt, 37.5 hour work week
  • Hours typically Monday-Friday during business hours; staff must be flexible in response to the needs of the target population served and occasionally work evenings or weekends if needed.Additional Information
