Job Description:
Encompass Health is seeking a dedicated Pharmacist to join our
healthcare team, focusing on providing top-notch pharmaceutical
care in the rehabilitation and home health/hospice settings. In
this role, you will be instrumental in helping patients regain
independence after serious illness or injury by managing medication
therapy and improving overall patient outcomes. This position
requires a detail-oriented individual who can work collaboratively
with other health care professionals to ensure the highest
standards of care. Review and execute physician’s prescriptions
checking their appropriateness and legality Organize the pharmacy
in an efficient manner to make the identification of products
easier and faster Maintain full control over delivering, stocking
and labeling medicine and other products Provide specialized
services to help patients manage conditions such as diabetes,
asthma, smoking cessation, or high blood pressure Advise on the
correct administration of drugs Assess the results of
pharmaceutical treatments Collaborate with healthcare professionals
to ensure optimal patient care Educate patients on possible drug
interactions and side effects Keep accurate documentation of
medication plans and patient progress Strong knowledge of
medication and its effects on the human body In-depth understanding
of legal guidelines for prescriptions and compounding Excellent
communication and interpersonal skills Proficient in technology
used for pharmacy management software and data entry
Detail-oriented with strong organizational skills Medical Insurance
Dental Insurance Vision Insurance 401k Valid and active Pharmacist
license Certification in Basic Life Support (BLS) Optional
certifications in Geriatric Pharmacy (CGP), Psychiatric Pharmacy
(BCPP), Oncology (BCOP), or a similar specialty depending on
patient needs
