We are seeking a Family Medicine Physician who is ready to step away from insurance-driven medicine and practice high-quality, relationship-based care in an innovative healthcare model. Responsibilities Deliver a full range of acute, chronic, preventive, and comprehensive primary care services to patients of all ages, including routine physicals, health-risk assessments, immunizations, screening tests, and personalized counseling on maintaining a healthy lifestyle Provide comprehensive care for a defined panel of Direct Primary Care (DPC) patients, utilizing clinic resources and materials to deliver high-quality, relationship-based care Diagnose, treat, and manage acute and chronic medical conditions with an emphasis on prevention, longevity, and whole-person health Respond to patient calls, correspondence, and requests, including in-office visits and telemedicine, in alignment with the 24/7 unlimited access principles of Direct Primary Care Practice as a member of an interdisciplinary healthcare team, providing care through direct treatment, consultation, collaboration, coordination, and referrals as appropriate Supervise, mentor, and collaborate with Nurse Practitioners and clinical staff as applicable Utilize evidence-based medicine, lifestyle medicine, and medical nutrition principles to prevent and manage conditions such as cardiovascular disease, diabetes, hypertension, obesity, and other chronic illnesses Develop and oversee individualized treatment plans, including preventive care, chronic disease management, and wellness optimization Maintain accurate, complete, and timely patient records.
