Clinical Care Manager/ Case Manager

Job Scope: Clinical Care Managers provide clinical oversight and
case management services to Colbert and Williams class members
transitioning out of nursing facilities in the Northwest Suburbs
and Lake County. Provide assessment, care coordination, and
clinical community-based interventions for mental health and/or
physical health needs. Treatment services are delivered at flexible
times that meet clients' needs and in settings that offer the best
potential for positive outcomes, including in hospital and nursing
home centers, in-home, and in community-based settings.
Proof of COVID-19 vaccination required. Accommodation for medical
and religious exemption will be considered. Weekly testing required
for approved vaccination exemptions.
Meet regularly with Class Members at assigned nursing facilities in
Chicagoland, primarily in Northwest Suburbs
Oversee and coordinate all service activities for Colbert and
Williams Class Members transitioning out of nursing facilities
Complete referral and intake into Mental Health services at KYC
Provide comprehensive, integrated assessments and integrate all
facets of clients' lives including mental/emotional health,
physical health/wellness, employment and education, social and
family relationships, substance use/abuse issues, spiritual,
community-integration, and housing needs
Create comprehensive wraparound service plans for class members,
using input from Class Members, SMHRF/NF staff, family and
guardians while focusing on a Class Members needs, desires, and
preferences
Provide direct service including skill building for Activities of
Daily Living and individual treatment to address trauma, substance
use, and other areas
Provide post-transition monitoring to Class Members for 18 months
and meet minimum in-person visit schedule to ensure all services
are being accessed and delivered
Administrative Responsibilities
Build and maintain relationships with nursing facility staff to
facilitate access to information and class member involvement in
comprehensive transition program
Interpret and communicate, at the service delivery level, Center
policies and procedures
Meet or exceed productivity target
Participate in orientations, all staff trainings, and training and
development seminars
Submit, in a timely and accurate fashion, schedules timesheets,
mileage and other reports required by the Center's policies and
procedures
Administrative Responsibilities

Masters Degree from an accredited College or University in Social
Work, Psychology, Counseling or closely related discipline
preferred (Qualified Mental Health Professional (QMHP) under
Illinois Rule 132/140). Bachelor's degree with significant clinical
experience may be considered.
Preferred 2 years of experience working with adults with serious
mental illness and/or chronic health conditions or complex physical
health and mobility needs
Preferred 1 year experience working in non-traditional treatment
environments , residential, nursing facility, hospital, community
settings
Applicant must have a valid IL Driver's License, proof of
insurance, and daily access to a reliable well maintained
vehicle
Basic computer and smartphone proficiency, knowledge of Word,
Excel, typing skills, and internet skills; Teams, Slack, etc), and
Electronic Health Record systems
